Your Medical Information

Sharing Your Information To Improve Your Care

We have signed up to a new system which will allow health and social care professionals from outside the GP practice directly involved in your care to view the most up-to-date information about you. This allows the professionals caring for you to more fully understand your needs and means you only have to tell your history once. For more information please ask for a leaflet or see the leaflets in the links below.

You will be asked for your consent whenever a professional outside the GP practice wishes to view your record. If you do not wish your information to be shared in this way please let us know. Accessing Medical Information Online

You can now access parts of your medical record on line.

As well as being able to book appointments and order prescriptions on line you will now be able to see parts of your medical record, however you will need to contact the practice and ask for this to be activated for you.

Sharing Information

From time to time annonymised medical information from our records may be shared with other health organisations to help develop better services.

With your consent, other health care professionals, such as A&E Doctors, can access information from your records, such as any medications you may be taking or any allergies you may have.

NHS England and your medical information

NHS England has developed a new system to enable them to plan NHS Services better, to improve the services offered and to provide the best possible care for everyone. This means that confidential information from your medical records along with your postcode and NHS number is being sent to a secure NHS computer system.

You have a choice

If you are happy for your information to be used in this way you do not need to do anything. If, however, you would like to prevent this from happening or want further information please contact the practice.

How your NHS records are used

The practice is taking part in a new NHS service that helps your family doctor (GP) identify whether you need more help to manage your health. This service is called "risk profiling".

Using information from your health records, a secure NHS computer system will look at any recent treatments you have had in hospital or at the surgery and any existing health conditions that you have, and alert your doctor to the likelihood of a possible future hospital admission. The clinical team at the surgery will use the information to help you get early care and treatment where it is needed.

The information will be seen only by qualified health workers involved in your care. NHS security systems will protect your health information and patient confidentiality at all times.

If you don't want your information being used this way, or have any other concerns please let the receptionist at the practice know.
