Register with us

Anyone living in the practice area can register with our practice even if they are already registered elsewhere. The registration process is simple and our friendly and helpful reception staff will help you complete the forms and arrange for your medical records to be transferred. Please see below for some frequently asked questions:

1. Who can register with the practice?

Any patient who lives within the practice boundary can register with us. You can check if you live within our catchment area by entering your postcode below. Alternatively, please check with our reception staff.

2. Is the process difficult?

No. All you need to do is to fill in a registration form, which our staff are more than happy to help you complete. Once this is done we will do the rest. You do not need to inform anyone else about this change, we will arrange for your medical records and all your information to be sent to us.

3. What will happen when I register with you?

Once your registration is complete we will receive your medical records from your previous GP. If you need to attend for any checks at the practice we will contact you.

4. What about my repeat medication?

Please bring in a list of your current medication, normally you will have a reorder form which will list all of these. If not please bring in any boxes/bottles you may have. This will allow us to issue any medication you need before we receive your medical records.

Please complete these fully and bring into the surgery.

The practice catchment area includes the villages of Ardsley (S71), Great Houghton (S72), Little Houghton (S73), Darfield (S73) and Low Valley (S73). Please check with the practice if you are unsure of whether you reside in the practice area.
